Fix linking to certificate in email
When the email notification about someone requesting to be tax-exempt is sent out the link to the uploaded certificate breaks whenever there is a space in the filename.

Approve / disapprove tax execotion links not working.
I clicke on the approved tax execnption link in the message request form and it does not work. Please fix this.

Allowed Upload File Types needs to be case-insensitive
When I enter Allowed Upload File Types as “doc,docx,pdf,rtf,txt,jpg,tif,tiff,jpeg,png,bmp” but then try to upload a JPG file, I get a “file type not allowed” error. The logic should not be case sensitive.

{user_name} in subject is broken
For whatever reason the subject line says “{user_name} New Tax Exemption Form Submitted” instead of the user’s real name. It’s displayed properly in the message body however.

Add fields to plugin.
Your plugin could use some tweaking. No account holders name or number. No name field No company name fields No address fields. No state fields. No City fields. No zip code fields. For improvement of this plugin
1. Better if there’s a setting where if the status is approved, the details from users end (not admin) the fields must be disabled. 2. From the admin side, there should be a notification once someone has submitted a tax exemption without going to the email.
