If you combine this plugin with Order Delivery Date Plugin you should be able to hide the general delivery date picker if you activate multiple shipping adresses in the cart.

The CSV import of the plugin does not work, since after the import WordPress does not go back to the product page to actually save the importet shipping rates.It stays on the “Import complete” page. Therefore making the complete CSV useless.

When you import shipping rates for an individual product, they are appended to the rate(s) already there. So you cannot import rates to overwrite the current rates even if they’re the same county/state/zipcode.
